This lady had a combination of genetic alopecia and some cosmetic surgery that caused her to have deep bald recessions at the temples causing a decidedly male appearing hairline. Her hair is medium to fine. She had two follicular unit hair transplant sessions, eight months apart. The first session consisted of 675 grafts, mostly single follicular units. They were placed to re-create the hairline in the bald area plus overlapping into the pre-transplant thinning hair. The second session consisted of 651 grafts. This time, two thirds were multiple follicular unit grafts to provide coverage behind the new hairline.We also placed more singles at the hairline. The reconstruction looks natural. When transplanting totally bald scalp with fine hair, especially in women, more than one session may be needed. The photo on the right was taken seven months after the second session.
